The importance of mezoterapie for hair loss. In general, mesotherapy is the superficial manipulation of the skin in such a way that pharmacologic agents are pushed into the mesoderm through multi-injectable means. The aim is for the pharmacologic topical agents to be directly delivered to the hair follicles. It was originally developed by a French physician in the year 1952. What has now been rolled out for hair is variously known around the world as intradermotherapy, intradermal therapy, intracutaneous injection, or in simple words, mesohair / hair mesotherapy.

Hair loss happens due to unhealthy diets, stress, hormonal imbalances, and the most common usage of hair rebonding and chemicals. More advanced mesotherapy nowadays has made it possible to insert more vitamins, minerals, proteins, etc. into the hair roots during the hair transplantation procedure. It is done using PRP treatment, which is very helpful during hair transplants to lessen hair transplant trauma and result in good hair roots. Advancements in this latest science have shown a significant ability in hair regrowth using only local anesthesia. It is an office-based procedure into the layer of the skin derived from the middle layer of the skin. The technique used is thin microneedles that are modified accordingly; single-point and nappage injections are used to provide aesthetic or minor improvements in skin texture and scalp hair regrowth. This procedure is popular for patients with thinning hair. No special precautions are required after treatment, and patients can resume their routine without any problems.

What are the Procedures and Techniques?

The hair mesotherapy treatment is a procedure performed in a day clinic. The number of sessions is variable depending on the stage of hair loss. Although there is no consensus, it seems that the ideal is to perform an initial treatment of four sessions with a frequency of two weeks between them. Subsequently, maintenance sessions are recommended every 2-3 months. Prior to the planning of the mesotherapy sessions, all patients must undergo a consultation where they are assessed because it is important to individualize each patient’s treatment. Part of this consultation is an assessment of possible health problems that can contraindicate the treatment as well as the prescription of any medication that the patient is taking at the time of the treatment.

For the injection technique, the main injection methods available are syringe and needle, pressure injection gun, dermaroller, micro needling, and intradermotherapy gun. The syringe and needle is the “classic” technique, while the pressure injection gun allows for a constant pressure injection rate. The derma roller is an instrument with needles attached in the areas of the active part that penetrate the skin to a combined depth of 1-3 mm. Microneedling is performed by applying a dermaroller with a silicone facial peel mask. The intradermotherapy gun is a new technology that, unlike the classic techniques, penetrates to different depths. All these techniques are effective, and dermaroller and microneedling probably favor better diffusion of substances due to the skin microtrauma they provoke, which leads to an inflammatory response. During these procedures, the use of gloves and skin antiseptics is mandatory, and all materials and equipment should be sterile and disposable, as well as respecting each country’s hygiene standards in order to guarantee the safety of the treatment. The substances to be injected are defined by professional experts. Mesotherapy for hair loss is a dermo-cosmetic, natural, minimally invasive, and non-surgical procedure, where a wide variety of pharmaceuticals specially formulated for each individual case are administered intradermally through microinjections of about 0.01 to 0.05 cc, which permits an excellent diffusion of the product in the area. The compounds are of different types, and those that are most used are vitamins, mainly those of the B and H complex and vitamins A and E, minerals such as trace elements and silicon, amino acids, nucleic acids, hyaluronic acid, and growth factors. It is important that the concentration of each component is suitable. For instance, in adults, we recommend that vitamin D is not included, while growth factors can be incorporated. Furthermore, substances such as deoxycholic acid or mannitol should be avoided. The concentration and dosage of the product depend on the patient and whether they are receiving other concomitant treatments. It is said that no hair mesotherapy treatment works if no holistic support is given, including functional medicine, psychological help, and stress management.

How Mesotherapy Helps Hair Growth

Mesotherapy is an injectable treatment wherein nutrients, growth factors, vitamins, and other bioactive substances are administered into the superficial layers of the scalp in order to promote hair growth and slow or reverse hair loss. Mesotherapy directly delivers ingredients that are essential for healthy hair and scalp and promotes vascularization and follicular metabolism. This enhances the delivery and effectiveness of other treatments and hair transplant results as well.

There are several mechanisms by how mezoterapie helps hair growth. First, consistent delivery of antioxidants and essential nutrients to the front of the scalp provides a shield around healthy, terminal hair follicles, limiting the hair shifts from anagen to a telogen phase. Second, mesotherapy is thought to help reduce inflammation and create an environment where hair can grow. Chronic inflammation from oxidative stress damages stem cells in the bulge of hair follicles, which can interfere with the hair cycle. By stimulating these cells and even increasing their number over time, mesotherapy further encourages healing of the scalp. Lastly, increased blood flow due to mesotherapy also helps deliver more nutrients to the site of injection, promoting healing and further hair growth. When ingredients are compounded to create a mesotherapy solution, the cumulative effect of the combination and the self-healing nature of the body creates a better response. Given these multiple mechanisms of action, we recommend tailoring injections according to your patient’s specific condition. The first step is to understand which products contain the right ingredients at the right concentration and that are safe to use. For example, biotin, vitamins B1, B2, B3, B5, B6, and B12, amino acids, and trace elements, such as magnesium, minerals, and dexpanthenol, exceed the minimum effective dose and have good safety profiles. A best practice is to use a single-vial cocktail where one high-quality base is enriched with a combination of nutrients designed to address hair aging.

Is it Safe to Regrow Hair with Mesotherapy?

The effectiveness of hair regrowth with mesotherapy has been reviewed in clinical studies as well as by reviewing other studies in the literature. Various studies have evaluated the positive outcomes in terms of hair regrowth, of mesotherapy alone or in combination with medical therapy, in alopecia areata, female pattern hair loss, male pattern hair loss, and telogen effluvium. A study showed that subjects with dermatosis and healthy subjects undergoing 16 sessions of mesotherapy showed 93–98% improvement in their skin and 100% patient satisfaction. Four randomized studies compared mesotherapy with minoxidil and found that mesotherapy had better efficacy than minoxidil, with 0% vs. 45% improvement with mesotherapy alone and minoxidil alone, respectively.

The causes of hair loss are multiple. Treatment of hair loss suitable to the cause of hair is important, so different modalities need to be considered for the treatment. Hair regrowth or hair restoration therapy is one of the common effects of hair mesotherapy treatment.

The treatment is safe and effective in three main types including:
(1) alopecia areata,
(2) female pattern hair loss, and
(3) male pattern hair loss.

Other studies have shown that mesotherapy can also be effective in other hair loss conditions due to inflammation or poor perfusion, such as hair loss due to telogen effluvium and hair loss in inflammatory scalp conditions. It is therefore suggested that mesotherapy can cause the regeneration of hair follicles that are not feasible to grow in these conditions. How mesotherapy helps hair growth has been shown to be safe with few side effects. In some cases, such as hair loss due to inflammation, it is necessary to consider contraindications in the form of swelling and inflammation on the scalp. In some hands and/or inexperienced doctors, intra- and extracellular migration of therapy can cause side effects such as lipodystrophy and fibrosis. In general, medical doctors, dermatologists, and trichologists who are skilled in this field can administer mesotherapy. A comprehensive examination and laboratory tests are required to determine the causes of hair loss before treatment is done. Long-term results can be achieved from various studies showing that the required maintenance may start with once a month and later as needed. However, in the hands of experienced hair regrowth with mesotherapy practitioners, and when used in conjunction with the appropriate treatment, hair that comes back is generally permanent.
